Finding Unique Services by Individualizing Opportunities and Needs (FUSION)The purpose of the FUSION program is to enhance and encourage community involvement thus allowing individuals to become more active members of their communities through social and skill development and support. The program is designed to take a unique look at ways each individual can utilize community resources based upon their own personal interests. Staff will aid the individual to define their areas of interest, prioritize what is important to them, determine available supports, and enlist this person’s personal network team in assisting the individual to become an active and integral member of their community. Some individuals currently involved in the program have developed relationships within their chosen organization including Clinton County Humane Society, Chakota Therapeutic Horse Farm, Breese Junior Women’s Club, the Knights of Columbus, and others.
