Adult Home Based Services and Supports

The Adult Home Based Services and Supports program provides individuals assistance to reside in their own home or their family’s home by providing limited financial support from the Illinois Department of Human Services. This is a self-directed program allowing the individuals to purchase services based on their individual desires and needs. Community Link provides service facilitation to aid the person in locating services and resources chosen. Options of services and supports that may be purchased include day programming, personal support workers, transportation reimbursement, therapies, or home adaptations which will allow the person to continue to live at home.

Community Link's Service Facilitator will assess the needs of the person and with the individual, their family, and independent service coordinator, develop an Individual Service Plan (ISP) to address those needs and preferences. The Service Facilitator will monitor progress of the plan.

This program is a Medicaid Waiver based program. Persons eligible for the program must meet the requirements set by the Illinois Department of Human Services.
