Enrich Home and Community InvolvementCommunity Link believes that all persons with developmental disabilities, regardless of age, should live and actively participate in a community that values their presence and contributions. Community Link provides families and young persons continual support in order to maintain a family unity, enhance caregiver skills, and prevent out of the home placement. Home Based ServicesThe Child Waiver Program, offered through the Illinois Department of Human Services, assists families in keeping their loved ones in their own home rather than seeking a more restrictive environment and allow families to manage their fiscal resources. The resources are managed by the individual and/or family with Community Link’s First Step program being the Service Facilitation Agent and the family being the managing employer. Service recipients in “Children’s Waiver” have their needs assessed by a Qualified Service Professional (QSP). An Individualized Plan of Service (ISP) is then developed to address the indentified needs. The target population for Community Link’s Children’s Waiver program is individuals who are 3-21 years of age with a developmental disability. Screening, referral, and eligibility determinations are made by a pre-admission screening agency. For more information contact the Service Facilitator at BrandiS@commlink.org. |
